The Enrollment Process

Submit Online Application 

We also require these items to complete your child’s application:

  • Birth Certificate
  • Health Forms (two forms)

 

  • Florida Certificate of Immunization DH 680 (or DH 681religious exemption)

 

  • School Entry Health Form – DH 3040

The DH3040 is two pages, a doctor checklist and parent checklist. Both pages must be provided.

Adjusted Gross Income – Your income is required so that we can calculate your

“Parent Contribution” of your overall tuition. Please provide your 1040 Tax Form.

Your adjusted Gross Income is line 11 of the 1040 Tax Form.

Free - Reduced Meals Application Form - We will this form for you to complete.

ELC Application

ELC (Early Learning Coalition)

You must complete the ELC application if you will be using our afterschool programming services. Our Afterschool program begins at 3:45pm
